The Direct Deposit Audit Form has been renamed to the “Annual Pensioner Information Form.”
On June 28, 2024 the Fund Office mailed a 2024 Annual Pensioner Information Form (“the Form”) to each Pensioner and Beneficiary for completion. The Form was attached to your 2024 Annual Pensioner Statement. The Form is a time-dated document due in the Fund Office no later than July 31, 2024.
Your 2024 Annual Pensioner Information Form is required by the Pension Plan in order to provide you with an opportunity to forward changes to your address, telephone number, or banking information. The Form also allows the Fund Office to obtain your current information required for the mailing of your T4A at year end.
Your Annual Pensioner Information Form must be completed in full, signed, dated and witnessed by anyone over the age of 18 years of age and is required even if you have no changes. If the Form has not been properly completed, it is invalid and a new Form is required. Article 7.2 of the Rules and Regulations of the Pension Plan confirms that “failure on the part of any Pensioner or claimant to comply with such request promptly, completely and in good faith shall be sufficient grounds to delay the payments to such person”.
Consequently, all Pensioners or Beneficiaries who have not returned their 2024 Annual Information Form when required, will have their pension payments suspended. Monthly pension payments that have been suspended will not recommence until the Fund Office receives and has approved the Annual Pensioner Information Form, at which time the payment will be set up for electronic payment commencing on the next available deposit date. No manual cheques will be issued.

Pensioners and Beneficiaries who commenced a pension on or after January 1, 2024 will not receive a Form until next year.
